      Job Summary

      The Supervisor, Care Management will monitor and supervise a group of associates in each region and/or market. The supervisor will assist the regional manager to monitor the team’s productivity and progress towards key performance indicators. The team leader will also assist with the execution of daily operations by overseeing staff training, team building exercises and performance reviews.

      Essential Duties & Responsibilities

      • Provides support to care management staff workflows and skill development in the use of care management data and reporting tools.
      • Facilitates all Care Management new hire and re-trainings especially when it impacts the work- flow of staff or will be implemented within the Care Management team.
      • The team lead will actively seek to resolve any concerns while adhering to the company policy and standards
      • Ensure department goals and objectives are being met by each employee and provide executive management with constructive feedback.
      • Responsible for motivating team members and assess their performance and evaluation.
      • Oversee the day-to-day team’s operation and performance.
      • Discover training needs, report to executive management, and create a plan to provide coaching.
      • Assist with periodic audits to monitor quality indicators and ensure performance metrics are being met.
      • Listen to team members feedback and resolve any issues or conflicts.
      • Maintains patient’s records by reviewing case notes, logging events and progress.
      • Coordinate discharge and post-discharge requirements; orienting and training family members; providing resources.
      • Creating and updating care plans for all patients actively enrolled in Care Management/Disease management program
      • Implementing cost-saving actions and improving Generic Dispensing rates (Target 91% GDR)
      • Reduce hospital admissions by coordinating outpatient care
      • Reduce hospital re-admissions (Target 10% readmission rate)
      • Assist with care management duties and coverage of centers when other care managers are out or on PTO

      Supervisory Responsibilities

      • Responsible for the direct oversight of the Care Management Coordinators team, including the management of timekeeping, performance, as well as the hiring and training of new associates.

      Critical Results

      • CR #1: > 90% of New Hires receive successfully on boarding and are adequately trained by 90 days probation period
      • Team lead will ensure > 90% of new hires receive at minimum satisfactory rating on 90-day performance review
      • CR #2: > 85% MRP completion for patients post discharge
      • New and existing Care Managers will complete > 80% of TOC cases in MDFlow for patients post discharge.
      • CR #3: Care Management Implementation and/or Integration
      • Successful implementation and integration of Care Management staff and workflows in new/acquired centers within the first 180 days.

      Best Practices

      • Monitor team performance through on-going evaluation and tracking of performance metrics as they relate to care management workflows (TOC, MRP, After hours, etc.)
      • Monitor new hire performance through on-going evaluation and tracking of performance at 30-, 60- and 90-day intervals.
      • Identify adverse trends with TOC and MRP to proactively implement effective strategies that address areas for performance improvement with care managers not meeting performance metrics
      • Provide input to annual team performance evaluations based on team performance
      • Provide input into the annual review of policies and procedures, the CM program results and outcomes, and future program goals and interventions
      • Oversee Care Managers productivity and ensure results are aligned with metrics and deadlines.
      • Support training for current and/or new acquisition staff to apply the correct workflows to effectively achieve department and organizational metrics.

      Education & Experience

      • 2-4 years of related work experience
      • MA, RN, LPN, Foreign medical practitioner, or equivalent medical experience.
      • Strong working knowledge of Microsoft Office applications
      • Excellent written and verbal communication skills
      • Strong organization and time management skills
      • Data collection, analysis, and interpretation skills
      • Strong organization and time management skills

      Knowledge, Skills & Proficiencies

      • Skill in operating phones, personal computer, software, and other IT systems.
      • Skill in oral communication
      • Ability to communicate with employees, patients, and other individuals in a professional and courteous manner.
      • Ability to pay close attention to detail and to ensure accuracy of reports and data.
      • Bilingual
      • Fluent in English and Spanish

      Physical Requirements

      This position works under usual office conditions. The employee is required to work at a personal computer as well as be on the phone for extended periods of time. Must be able to stand, sit, walk, and occasionally climb. The incumbent must be able to work extended and flexible hours and weekends as needed. Physical demands include ability to lift up to 50 lbs. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
