What to Expect (Job Responsibilities):
- Supervise daily operations of the clinical pharmacy outreach service to ensure coordination of Medication Therapy Management (MTM) services
- Act as a liaison between internal and external customers for issue resolution
- Ensure compliance with CMS guidelines and reporting requirements for the MTM Program
- Conduct comprehensive medication reviews and provide education to members and healthcare providers about medication interventions
- Assist with staff training and coaching, identifying training needs in collaboration with management
What is Required (Qualifications):
- Doctor of Pharmacy from an accredited pharmacy school
- 3-5 years of experience as a Clinical Pharmacist
- Active and unrestricted State Pharmacy License for workplace and plan location
- Current knowledge and expertise in clinical pharmacology and disease management
- Clinical skills related to medication therapy management
How to Stand Out (Preferred Qualifications):
- Ambulatory Care Residency
- Experience with Medicaid and/or Medicare programs
- Board Certified Ambulatory Care Pharmacist
- Completion of the American Pharmacists Association Medication Therapy Management Certificate Training Program or equivalent
