      What to Expect (Job Responsibilities):
      - Provide clinical guidance and oversight to care management team members within the RN Supervisor's licensure scope
      - Develop effective relationships with co-workers, direct reports, and community partners to meet organizational goals
      - Promote the importance of Enhanced Care Management (ECM) and Community Support Services to improve member health
      - Ensure compliance with regulatory requirements while developing workflows and processes for care management operations
      - Conduct monthly chart audits and review daily productivity charts to monitor team performance and quality of care

      What is Required (Qualifications):
      - Minimum of three (3) years of experience in acute care or Behavioral Health Care field
      - At least two (2) years in a lead or supervisory capacity
      - Associate's Degree in Nursing from an accredited institution required
      - Active, unrestricted Registered Nurse (RN) license issued by the California BRN
      - Valid California Driver’s license and valid automobile insurance with a driving record of no more than three (3) points

      How to Stand Out (Preferred Qualifications):
      - Bachelor’s degree in Nursing from an accredited institution
      - Certified Case Manager or other related certifications
      - Knowledge of services for members experiencing homelessness or serious mental illness
      - Skilled in evidenced-based communication strategies such as Motivational Interviewing
      - Experience working with multi-cultural communities and understanding social determinants of health
