Provide initial screening, intake and eligibility determination for rent assistance and other social service supports for individuals and families impacted by homelessness (financially/economically).
Connect with landlords on behalf of clients to ensure rent assistance to prevent eviction
Perform complex case management duties including but not limited to assessing, evaluating and documenting eligibility for Regional Long-Term Rent Assistance (RLRA) vouchers, rental assistance program services using a comprehensive intake application process, and other program specific paperwork to assist with housing stability.
Assist program participants through the housing process including identifying an apartment/home, addressing possible barriers to housing approval, and accessing funding for move-in cost
Provide ongoing home and community-based services utilizing strengths-based case management while embracing the philosophy of Motivational Interviewing, Trauma Informed Case and Housing First
Monitor services/supports via home visits, service provider visits and phone calls to ensure that they are receiving appropriate financial, medical, training, educational, transportation, and other identified/agreed to services/supports.
Meet people in their homes, residential settings, and employment settings to conduct monitoring
Ensure resources are being appropriately used to meet clients’ developmental disability needs. Extensive documentation and plan writing to develop services plans with appropriate levels of service and risk mitigation
Assist in coordination and implementation of specialized programs including but not limited to the supportive Housing Services Regional Long-term Rent Assistance (RLRA) program, Emergency Housing Program.
Bachelor's degree from an accredited college or university with major course work in social work, psychology/sociology, criminal justice or a related field. (Professional work at or near the level of this position can substitute year for year for the educational requirement.)
Three (3) years of increasingly responsible research, field investigatory or case management experience as a journey-level social services, court, criminal justice or corrections/protective services professional
Strong Background working with individual, and families who experience chronically homelessness

Must pass a criminal background check
Knowledge, Skill, Ability (KSA) Advanced knowledge and experience working with chronic homeless individuals living with other comorbidities, including addiction, behavioral health needs, disabilities, and other health conditions.

This specific position will not have a designated caseload. This is a KSA (Knowledge, Skill, Ability) position the ideal candidate should have advanced knowledge and experience working with individuals experiencing chronic homelessness. This position will provide intensive case management, Housing and rental assistance, and anti-poverty work related to Supportive Housing Services and Recovery efforts related to the housing crisis and adults with intellectual and/or developmental disabilities. This position requires HIGH PUBLIC CONTACT. The purpose of Supportive Housing Services Case Manager is to access housing, coordinate and assure the delivery of services and supports required by individuals with intellectual and/or developmental disabilities. Case Management will assist eligible individuals under their individualized plan in gaining access to needed housing, medical, social, educational, and other services.
As the Supportive Housing Services Case Manager , other duties will include:
