Job Description

- Directly overseeing and providing supervision, development, and coaching of the Enhanced Care Pathways (ECP) team and Outreach Staff, a total of approximately six (6) staff.
- Participating in and providing training for ECP training at New Employee Orientation a monthly basis.
- Serving as the point of contact for internal ECP referrals and assigning admits to the outreach team.
- Taking ownership of the ECP spreadsheet to ensure information is current and up-to-date from admit to discharge.
- Assisting staff with clients to stay involved in the day-to-day services as well as find opportunities for coaching and staff development.
- Facilitating team meetings on a bi-monthly basis to review success, barriers, and help keep staff informed of new developments within the team and the organization.
- Assisting the Program Manager with chart audits, data collection, and monthly reports for various funding sources.
- Bachelor’s degree in a social science field (as defined by the Department of Mental Health) required.
- Must qualify as a Community Support Specialist as defined by the Department of Mental Health.
- Three (3) years of experience in direct service provision in a mental health setting.
- An additional three (3) years of experience providing community support services to individuals with mental illness or substance use disorders in accordance with the community support key services function. Experience is specific to the adult or child/youth/adolescent population the individual will be supervising.
- A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work.
- Working knowledge of mental illness and substance use disorders.
- Excellent customer service skills with a recovery focus.
- Strong written, oral and interpersonal communication skills.
- Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as G-Suite.

The Diversion and Intervention Services team partners with law enforcement and courts with the goal of empowering officers and court staff with resources, education, and skills to better respond to persons with mental health issues, as well as to divert unnecessary arrests, court cases, or hospitalizations.
