      Main duties of the job

      As a Team Leader at Avery Healthcare, your focus will be to support the home management team in delivering high-quality, person-centred care by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff. You will be responsible for leading in the care planning process, assisting with resident discharges, monitoring the well-being of each resident, and practicing safe systems of work.

      Other responsibilitieswillinclude:

      • Leadingin the person-centred care planning processby implementing,monitoring,and undertaking monthly evaluations, completing care plan audits, developing action plans, andbeing responsible forcompletingthe action plans.
      • Assisting in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed, where appropriate.
      • Leadingwithmonitoringthewell-being of each resident, being aware of any changeimpactingcare and care planning andensuring that changing needs are met and written records are amended to reflect any variation.
      • Practicing safe systems of work across a range of tasks and moving and handling of residents, by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
