      Job summary

      You will be a registered Nursing Associate, and will have completed your Foundation Degree. You will work as part of a Primary Care Network/GP practice multidisciplinary team, providing safe, person centred care to improve the health of our population.

      In line with best practice and with support from the MDT, you will carry out various procedures to monitor and improve the health of patients, including physical observation, venepuncture, ECG recording, injections, and other procedures commensurate with your training and skills.

      You will work in collaboration with a range of other professionals including GPs, ANPs, practice nurses, district nurses, and other community services.

      You will be working as part of the Teesdale PCN team to support the patients at Barnard Castle Surgery, Woodview Medical Practice, Pinfold Medical Practice, Gainford Surgery, Evenwood Surgery and The Old Forge Surgery.

      Main duties of the job

      • Under the supervision and working in partnership with a registered nurse, provide and

      deliver a high standard of nursing care contributing to the ongoing assessment, providing

      and monitoring care based on evidence and working in partnership with the multi-

      disciplinary team.

      • Provide compassionate, safe and effective care and support to patients in a range of

      care settings.

      • Monitor the condition and health needs of patients within their care on a continual basis

      in partnership with patients, families and carers, ensuring appropriate escalation to a

      registered nurse or other professional when required.

      A group of 12 GP practices across Durham Dales got together, looking for a solution to help them provide the care patients needed, share resources and ideas, and be prepared for the challenges the NHS would face in the future.The answer they came up with was a formal collaboration between the dozen practices: the Durham Dales Health Federation.       • Maintain accurate, contemporaneous patient records in line with [PCN/practice] policy and the NMC Code.

      • Provide effective evidence based care for service users under the supervision of

      registered nurses and other professionals in order to perform non-complex activities, in line with clinical governance and professional standards.
