      The Opportunity:

      Overview:

      THIS WORK MATTERS!

      This is a Temporary Case Manager 2 Oregon Needs Assessment (ONA) to assess the level of functioning of individuals impacted by Intellectual and Developmental Disabilities, the ONA Assessor completes the Level of Care form and submits it to the state for approval through its electronic payment and reporting system. The Assessor will also complete the Support Needs Assessment Profile {SNAP) assessment tool for children and adult foster care rate-setting. This tool also identifies the support needs that will be addressed in the annual support plan.

      Temporary assignments are not eligible for benefits.

      As the Temporary Case Manager 2 ONA Assessor , other duties will include:

      • Review client files in Velma electronic system including eligibility documentation, prior service plans, Incident Reports, Risk Identification Tool results, and behavior support plans

      • Conduct client fact-to-face observations

      • Use information from document reviews and client observations to assist in interpreting the information provided by the respondents in ONA assessment interviews with the client, their family, guardian, and/or collateral contacts

      • Develop an accurate portrayal of consumer’s strengths, needs, choices, and risks

      • Consider all care options and settings, appropriate level of service, and risk mitigation plan

      • Complete additional client assessments required by law or county policy

      • Ensure compliance with state, federal and local rules, standards and guidelines

      • Enter and submit assessment into web-based application

      • Track, complete, and document NOPAs as needed

      • Attend state-required training

      • Email, phone, and in person communication with clients, family members, legal/designated representatives, support providers, and Service Coordinator

      Minimum Qualifications/Transferable Skills*:

      Education/Training:

      • A Bachelor’s Degree in Behavioral Science, Social Science, or closely related field from an accredited College or University

      • Three years of Human Services related experience, providing assistance to people and groups with issues, such as economical disadvantages, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing

      • Diversity and Equity: Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values

      • Must pass a criminal background check
