      What to Expect (Job Responsibilities):
      - Develop individual plans of care based on personal preferences and choices of individuals being served
      - Utilize community resources and other supports to enhance individuals' quality of life
      - Maintain ongoing contact with individuals and families to ensure plans of care are implemented effectively
      - Coordinate, select, and oversee supports for individuals and their families
      - Collaborate with a regional support team and inform providers about service gaps

      What is Required (Qualifications):
      - Bachelor's Degree in a relevant discipline and 1-year equivalent experience working with individuals with disabilities
      - Proficiency in MS Office Outlook, Word, Excel, and PowerPoint
      - Strong problem-solving, organizational, and communication skills
      - Excellent time management skills to handle multiple plans for multiple families
      - Valid driver's license and car for travel throughout the state of New Jersey
      - Bilingual preferred

      - 0-2 years of experience in disability services or a related field
