- Participate in the recruitment and interview process of therapy staff.
- Manage, orient, train and evaluate department employees to maintain high standards of performance and quality patient care.
- Coordinate treatment activity between disciplines and team members.
- Ensure staff are compliant with documentation; and they are provided with information, guidance and training via in-service training programs.
- Communicate effectively with administration regarding department programs, goals, objectives, risks and successes.
- Organize the creation and implementation of therapy groups, programs and services in line with CDC recommendations and/or current evidenced-based practice.
- Communicate with contracted agencies for provision of staffing and other therapy needs as required.
- Communicate therapy service changes to all therapy staff and IDT members to ensure optimal implementation.
- Generate and disburse monthly therapies reports regarding outstanding documentation, service deliveries, and order completion and accuracy.
- Assign patient scheduling and other responsibilities to staff as needed.
- Ensure the assessment, planning, preparation and implementation of individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients is completed.
- In conjunction with therapies staff, evaluate the effects of treatment at various stages and adjust treatments to achieve maximum benefit and to meet the patient's goals and needs.
- Manages the administration of manual exercises: instructing, encouraging and assisting patients in performing physical activities, functional activities, gait and daily-living activities and in using assistive and all applicable supportive devices.
- Manages the administration of physical therapy treatments: utilizing physical agent modalities-moist packs/heating pads, cold packs, TENS, EMS, ultrasound machine and traction, directing treatments given by assistants.
- Manages all rehabilitation activities and ensure activities are focusing on participant’s needs in order to improve their overall health and wellbeing.
- Ensure information is provided to patients regarding the proposed intervention, material risks and expected benefits, and any reasonable alternatives.
- Document in accordance with organizational/departmental guidelines and within the established time frames.
- Ensures assessments are completed in collaboration with physicians, nurses, social workers, and other pertinent IDT members to establish an integrated individualized plan of care.
- Participates in the fall prevention committee and ensures new strategies are implemented and are audited for effectiveness.
- Works with the outreach and enrollment manager to support and engage participants to participant in the yearly HOS-M Questionnaire. Strategize opportunities for completion of the survey in the center.
- Oversee the designing of home exercise programs. Maintains patient confidence and by keeping information confidential.
- Works in conjunction with leadership to develop and monitor operational budget and staffing requirements for the departments, and develop and review departmental policies.
- In accordance with policies and procedures, rules, and regulations, ensures a safe and clean working environment is maintained.
- Attends all mandatory meetings and trainings.
- Protects patients and employees by adhering to infection-control policies and protocols.
- Assures that appropriate supplies, tools and equipment are ordered, maintained, and available for use when needed.
- Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
- Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
- Observes each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
- Will support therapy services with assessments, treatments, and care plans.
- Other projects and duties as assigned.
- Requires the exertion of 50 pounds of force occasionally, up to 20 pounds of force frequently, and up to 10 pounds of force constantly to move objects.
- Will require 20% sitting, 30% walking, and 50% standing.
- May also need to move heavy equipment, transfer/lift patients and assist patient with walking and turning.
- Moving, lifting or transferring participants may involve lifting or pushing greater than 50 pounds, should be done with assistance as appropriate.
- Must be able to hear staff/clients on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
- Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
- Physical Therapist may also have to stoop, kneel, crouch, lift, and stand for long periods.
- May be required to travel to sites/program and special functions as needed.
- Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
- Work is in an office environment/rehab gym, climate controlled through central air conditioning, and outdoors, subject to weather conditions.
- Low risk for contamination when adhering to Universal Precautions.
- Will work with Rehab exercise equipment and Durable Medical Equipment.
- Will have some exposure to outside environment while traveling and/or working outdoors.
- Strong interpersonal skills and ability to work as a team member.
- Excellent oral and written communication skills.
- Ability to handle multiple tasks and meets deadlines.
- Computer literacy in Microsoft Word, Excel, Power point.
- Knowledgeable of proper body mechanics and lifting techniques.
- Patience and understanding of frail elderly.
- Must have one of the following qualifications:
- Doctor of Physical Therapy Degree;
- Grandfathered with a Masters Degree; or
- Qualifying physical therapy degree earned outside of the US which has been evaluated to the US equivalent to a degree in physical therapy
- Active and unencumbered Physical Therapist license issued by the State of California Physical Therapy Board
- Minimum of five (5) years of experience working as a Physical Therapist
- Minimum one (1) with the frail, elderly and disabled populations.
- Three (3) years of supervisory experience Therapy and/or Rehabilitation.
- Current AHA Certified BLS CPR.
Responsible for ensuring therapy services are developed and implemented at all therapy locations. Management and supervision of Physical Therapy, Occupational Therapy, Speech Therapy, and Recreational Therapy staff, actively engaging in employee relations matters. Ensure customer service standards are adhered to, and financial objectives met, through strong leadership and coordination with the Director of Physical Therapy and Rehabilitation Specialties and PACE Director of Center Operations RN.
Will work at our PACE Modesto Center, PACE Merced Center, and Modesto McHenry Specialty Clinic.
Schedule: Monday – Friday, working 8:00am – 5:00pm.
Compensation: $146,120.00 - $156,348.40 Salaried
