Job summary
The post holder will be a dynamic individual who will motivate health professionals to promote and develop nursing care in all areas of tissue viability; creating and maintaining a culture of effective patient and health care services. To promote pressure ulcer prevention and reduce patient harm.
The post is multifaceted as it has clinical, educational, audit, research and development components. The post holder will have skills and knowledge in the field of tissue viability, you will be required to work autonomously in performing highly skilled assessments of complex wounds and implement wound management and pressure ulcer prevention plans. You will provide expert advice and support to health care professionals within the Trust and across Mid Yorkshire Community to improve care delivery and outcomes with support from senior Tissue Viability colleagues.
Please note, the working hours for this post are 24 hours per week.
Main duties of the job
The post holder will act as an expert nurse and provide advice and intervention to all patient groups with tissue viability needs, promote a seamless service within a multi-professional framework and will contribute to the development of the tissue viability service.You will be responsible for the provision of specialist advice and support to Trust and non-Trust healthcare professionals in relation to Tissue Viability.Provide education and training on Tissue Viability, including wound management, pressure ulcer prevention, pressure relieving equipment and leg ulcer management
To discuss/demonstrate dressing choices/techniques for complex wounds at request of referring professional. Triage referrals in to Tissue Viability for wound assessment and pressure ulcer prevention.
Participate in the investigation of tissue viability related incidents within the Trust and assist in the setting and implementation of action plans
Liaise with acute Trust, community district nurses, residential homes, nursing homes, and other disciplines regarding the management, discharge and transfer of patients with complex Tissue Viability requirements.
The post holder will contribute to achieving the objectives of the Trust by implementing quality improvement strategies in relation to pressure ulcer prevention to reduce trust acquired patient harm.
Take an active part in the development of nursing in the Trust via participation in professional nursing forum and shared governance initiatives.
