      Job summary

      Demonstrate ongoing evidence of capability at masters Level across the 38 standards within the four pillars of Advanced Practice outlined in the Multi Professional Framework for Advanced Practice (Clinical practice leadership and management, education, research)

      Be able to meet the eligibility and enrolment criteria for the University of the West of England course and can provide the certification etc.

      Advanced Clinical Practitioners (Level 7) are experienced clinicians who independently manage the clinical care process from start to finish. Working across traditional health and social care boundaries, they combine expert clinical skills with research, education and leadership.

      Main duties of the job

      The successful postholder will:

      Demonstrate a critical understanding of your broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice including when working with complexity risk, uncertainty, and incomplete information.

      Work in partnership with individuals, families and carers', using a range of assessment methods as appropriate (e.g. of history-taking, holistic assessment, identifying risk factors, mental health assessments, requesting, undertaking and/or interpreting diagnostic tests, and conducting health needs assessments, physical examination, clinical measurements using advanced clinical skills, detailed medicines history, undertaking and interpreting range of tests / investigations.

      Exercise professional judgement to assess and manage risk appropriately, where there may be complex and unpredictable events and provide the required support to teams to ensure safety individuals, families and carers'.

      Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make positive changes, using Health Education England's Framework to promote person-centred approaches in health and care.

      To be responsible for the rapid assessment, diagnostics, and treatment of individuals within the service using an advanced level of professional accountability, autonomy, and judgement, underpinned by Masters level theory and experience. This includes highly complex decision-making across a broad range of differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses.

      To plan and manage complete episodes of care. This will be working in collaboration with senior medical colleagues and others when required. Delegating and referring as appropriate, to ensure timely, effective management to optimise health outcomes, in line with the evidence base and nationally recognised best practice.

      To play a key operational role and maintain a pro-active approach to managing, updating and leading clinical pathways.

      To promote and implement service and policy development and redesign, impacting beyond own area and other disciplines, informed by current best practice and evidence-base, in accordance with values-based care.

      To ensure high quality learning environments for all disciplines within a context of multi-professional education and training that is responsive to future changes and challenges. Reflecting & imbedding current trends & feedback e.g. Health
