      JOB TITLE: Transition Facilitator, Special Education

      TITLE CODE: T25600

      REPORTS TO : Coordinator, Special Education

      DEFINITION: Plans, coordinates, leads, trains, and participates in the provision of transition services to middle schools, high schools, and post-secondary students with disabilities. Ensures that all initiatives support the system-wide goals identified in The Compass: Our Pathway to Excellence.


      EXAMPLE OF DUTIES:

      Collaborates with stakeholders to provide transition planning services from school to post-secondary opportunities in an educational or work setting for middle and high school students with an Individualized Education Plan (IEP).

      Participates with IEP/transition teams in the development/review of transition plans, outcomes, services, and linkages to adult service agencies.

      Serves as a resource for transition teams, Developmental Disabilities Administration (DDA) personnel, coordinators of community services, Division of Rehabilitative Services (DORS) counselors, teachers, BCPS school staff, MANSEF staff and other stakeholders.

      Assists schools with maintaining compliance with all postsecondary transition mandates including MSDE State Performance Plan Indicator 13.

      Provides transition information and technical assistance to teachers, IEP team members, parents, students, and other interested parties in a variety of formats.

      Assists with student career interests, skills, and knowledge by providing training, coaching and feedback related to postsecondary transition planning for case managers, teachers, department chair, IEP chairs, parents, school counselors and career and technology education teachers.

      Arrange tours related to career interests or agency opportunities; arrange for speakers from adult agencies to visit schools or present to parents.

      Provides professional development to school-based staff, IEP teams, parents, and community members on transition topics.

      Assist School to Career teachers with students with IEPs by providing resources and consultative services on work accommodations.

      Develops collaborative relationships with external partners and exchanges information to benefit students with IEPs in a seamless transition from school to post-secondary opportunities in the world or work, education, and training.

      Provides linkages to appropriate agencies by obtaining parent permission to release records with applications for services.

      Designs, distributes, compiles, copies and disseminates forms to collect data related to the progress of student work-based learning experiences, and preparation for transition to postsecondary options.

      Supports planning and implementation of transition forums/fairs for parents and student's county wide with agency representatives attending.

      Participates in outside agency committees as needed to enhance postsecondary transition knowledge of ever-changing policies and procedures.

      Completes and submits monthly and quarterly reports pertaining to enclave and work-based learning.

      Provide postsecondary training presentations to school staff, parents, and students as needed.

      Performs other duties as assigned.


      MINIMUM QUALIFICATIONS:

      Education, Training and Experience:
      Graduation from an accredited college or university with a master’s degree in special education or a related field. Three years of satisfactory experience working with middle or high school special education students either in a teaching or support services position. Experience in vocational education/school to work programs is preferred.

      Licenses and Certificates:
      Possession of, or eligibility for, a Maryland Advanced Professional Certificate in Special Education.
      Possession of, or eligibility for, a MSDE endorsement for Work Based Learning preferred at the time of hire. The Work Based Learning endorsement must be obtained within one year.

      Knowledge, Skills, and Abilities:
      Thorough knowledge of inclusive educational practices, IDEA guidelines, IEP development, LRE (Least Restrictive Environment), and compliance with federal and state mandates including knowledge of transition service requirements and labor laws. Demonstration of outstanding oral and written communication skills. Demonstration of expertise and experience in assessment/data analysis and instruction ages three through post-secondary. Demonstration of strong human relations, communication, IEP team facilitation and organizational skills. Demonstration of strong computer/technology skills.

      PHYSICAL AND ENVIRONMENTAL CONDITIONS:
      The work of this class is generally performed in schools, office environments and in the community. Work requires the ability to travel to schools, community providers, worksites, and transition events.

      Requires attendance at after-school and evening meetings and events. Requires attendance at staff developments.
