What to Expect (Job Responsibilities):
- Coordinate care transitions for members experiencing significant health changes
- Complete post-discharge questionnaires and ensure medication understanding
- Monitor members for alerts or changes in condition and facilitate necessary interventions
- Conduct outreach calls and manage post-discharge follow-up
- Facilitate interdisciplinary care team meetings to discuss service needs and transitions

What is Required (Qualifications):
- Minimum of 2 years of LPN nursing experience
- Unrestricted and active compact LPN/LVN licensure in state of residence
- High School Diploma or equivalent (must be an LPN)
- Exceptional verbal and written communication skills
- Ability to multitask and adapt in a fast-paced environment

How to Stand Out (Preferred Qualifications):
- 3+ years of LPN nursing experience
- Experience in discharge planning
- Advanced proficiency in Microsoft Word, Excel, and Outlook
- Self-motivated, detail-oriented, and tech-savvy
- Strong organizational skills
