      Summary

      Starting Salary $54,000.00

      At Pace we transform students’ lives through academic and counseling programs based on a gender-responsive, strength-based and trauma informed framework. Team members create a safe place for students to learn, process, heal and begin again.

      The Transition Services Specialist is responsible for providing services for middle and high-school students who are active in the day and Reach programs and aftercare/follow-up services for transitioned students. The position will focus on case-management and academic, career and financial aid counseling to participants to encourage them to graduate from high school, pursue post- secondary education and/or a viable career path.

      Qualifications and Education Requirements

      Required

      · Bachelor’s Degree or higher from four-year college or university or Associates degree and equivalent relevant work experience. Degree/Major in Social Work, Psychology, Mental Health, Counseling, or a related field

      · Minimum two to three years’ related experience Understand culturally relevant needs of diverse populations

      · Knowledge of case management and behavioral health practices

      · Excellent individual, group and family counseling and crisis management skills Proficiency in computer skills, database management and/or office software Current Florida driver’s license

      Preferred

      · Master’s degree from an accredited College or University

      · Knowledge of laws relating to children and families Knowledge of state and school districts policies Ability to incorporate group facilitation skills Experience with at-risk youth

      Roles and Responsibilities

      · Helps students develop a plan for transition that corresponds with their academic needs and interests. Provides on-going case-management, guidance progress monitoring and support in assisting students to achieve individual transition goals and objectives.

      · Reviews students’ academic records to ensure current information and accuracy.

      · Assists in the review of credits, GPA, and other required academic measures for successful transition. Provides career counseling and assistance to students in the initiation of job interviews, job placement, and intervention when a job is lost; provide educational and vocational materials.

      · Administer career exploration and aptitude assessments.

      · Assists with coordination of college tours.

      · Participates in care review meetings before for the purpose of transition planning discharge to plan for ongoing intervention.

      · Establishes positive relationships with outside agencies, educational and vocational institutions to promote student inclusion.

      · Monitors and records the progress of students who have transitioned or settled into life roles for a period of one continuous year; documents all areas of progress assessment in accordance with Pace Center operating procedures and regulatory agencies.

      · Adheres to established budget guidelines; manages existing resources to minimize cost wherever possible.

      · Collaborates with student’s Pace care team/advocacy team regarding post-transition services and ensures these services are communicated to caregivers prior to the transition-planning meeting.

      · Assists or facilitates workshops and conduct support groups to discuss issues that impact student success

      · Provides crisis intervention when special needs or concerns arise.

      · Coordinates intervention with program counselor and/or appropriate referral agencies.

      · Constantly evaluates progress of post-secondary, career and/or vocation goals and reevaluates long term/short goals.

      · Other duties as assigned.

      · Availability to work evenings and weekends as required to fulfill workload requirements

      · Must be able to travel by automobile, plane, train, etc.; occasional overnight travel may be required

      Work Environment

      The work environment characteristics can be stressful, is fast-paced and requires alertness and flexibility. The noise level is usually quiet; however, noise can accelerate to moderate and high levels.
