      What to Expect (Job Responsibilities):
      - Manage inbound notifications related to transitional care events from various sources
      - Conduct outreach to inpatient and post-acute facilities to gather information on admission, discharge dates, and discharge plans
      - Request and obtain medical records, routing documents to clinical staff for review, and uploading them into the electronic medical record platform
      - Schedule and confirm post-discharge appointments with patients using the electronic medical records platform
      - Coordinate with care teams, facilities, payers, and community organizations to meet patients' transitional care needs

      What is Required (Qualifications):
      - High school diploma required; college degree preferred
      - Strong computer skills, including basic knowledge of Microsoft Excel and electronic medical records
      - Experience in a customer service setting, preferably in healthcare
      - Professional phone etiquette and a high level of integrity
      - Fluency in Spanish, Polish, Russian, or other languages spoken within the communities served (where necessary)
