SUPERVISORY RESPONSIBILITIES

SUMMARY
Transitional Support Advisor will be responsible for carrying a caseload of students enrolled in the Valley Apprenticeship Connection program (VAC). Transitional Support Advisor will work in a training and employment environment and be held responsible for assigned students’ outcomes.
Assist in the fulfillment of the agency’s vision, “For Those We Serve,” by empowering individuals who thrive as healthy, self-sufficient and contributing members of our communities.
DUTIES AND RESPONSIBILITIES/ESSENTIAL FUNCTIONS
Identifies and addresses all service needs of a student to achieve contractual obligations and successful outcomes, conducting individual and/or group assessments, development of career plans, assisting with community resources to address barriers and providing career counseling.
Maintains direct communication with various contractual partners i.e. Union Trades, Community Colleges, Parole Agents, Probation Officers, and Instructors.
Determines program eligibility of potential clients.
Interviews assigned students to determine skills, abilities, education and previous work experience.
Maintains integrity, confidentiality and fidelity of the program at all times.
Monitors the youth’s progress towards their stated career and training goals and make adjustments as needed to facilitate the best possible outcome for each individual youth.
Develops individual career plans.
Records case note documentation in a timely manner.
Maintains data in a common case management system.
Cultivates sound working relationships with collaborative partners such as Union Trades, State Center Community College District, Economic Development Corporation of Fresno County, State of California and other partners.
Provides career counseling and case management services.
Provides drug counseling and referral for students in need of substance abuse treatment.
Monitors the students’ progress towards their stated goals and adjusts as needed to facilitate the best possible outcome for each individual student.
Accurately completes all student’s narratives to reflect participant’s progress within 24 hours from the actual event and/or contact.
Seeks additional community resources to meet participant needs for food, clothing, housing etc. Assists participant with referral and/or enrollment assistance in other activities necessary for the participant’s success.
Monitors and manages the ongoing progress and exit of the students according to established performance requirements.
Motivates students for full participation in all scheduled programs.
Students in focus groups and strategizing teams as needed for continuous quality improvement.
Responsible for managing participant exits to maximize client flow and youth performance outcomes.
Travels for agency business using personal/agency vehicle.
Maintains regular attendance and punctuality.
Works evenings and weekends as required.
Performs other duties as assigned.
QUALIFICATIONS

EDUCATION:
High school diploma or GED.
Bachelor’s Degree in Social Work, Psychology, Criminal Justice or closely related field required. Additional relevant work experience may substitute required education on a year-by-year basis.
EXPERIENCE:
One (1) year work experience in a case management, criminal rehabilitation or closely related field preferred.
OTHER QUALIFICATIONS:
Alcohol Other Drug (AOD) certification preferred.
Proficient in Microsoft Office Suite and related business software.
Effective presentation, written and verbal communication skills.
Ability to work effectively with diverse target population from target populations specified in program grants.
Demonstrated ability to write reports and motivate students.
Maintain a valid California driver’s license, reliable transportation, and adequate auto insurance as required by State law.
Bilingual in English and Spanish preferred based on the needs of the clients and the program.
COVID-19 vaccination required within 60 days of hire, unless approved for religious or medical exemption.
