Job Description

Calling members to check in after discharge to see how they are doing, and that they have received the assistance they need
Work with affiliated vendors to coordinate services and care for member, to include transportation, DME, meals, home health care and more as appropriate.
Assisting with scheduling of post discharge PCP and Specialist visits
Collaborate with members, providers, and caregivers to ensure a positive outcome.
Working hand in hand with the other members of the TOC team to address member needs towards team based goals. This may include interdisciplinary huddles.
Participating in and assisting with pilot programs
Coordinating with medical providers and facilities to access necessary discharge records
Following escalation processes to clinical team to ensure members get needed evaluation and care
You love helping others improve their health and navigate a complex healthcare system with compassion, empathy, and warmth.
You are comfortable performing care coordination for a predominantly elderly Medicare population.
You are able to ease the stress and anxiety of members during difficult times.
You have a natural ability to successfully communicate and interact with different stakeholders in a member’s life from doctors to pharmacies to nurses to caregivers.
You are comfortable working with members over the phone.
You are an entrepreneurial self-starter who enjoys taking the initiative but is also a great team player willing to work hard to achieve a common aim.
You are comfortable engaging and learning new technologies including electronic health records and different computer platforms, operating systems and programs (Google Chrome, Google Sheets).
You have a passion for supporting the delivery of healthcare that we would want for our own family.
You're excited about being part of a program that is designing and implementing new process changes - frequent change excites you instead of making you feel stressed.
Minimum educational experience: high school diploma or GED equivalent
2+ years in healthcare setting, managed care, home health, community health highly desired
Associates degree in healthcare related field preferred
Fully bilingual (English/Spanish). (Preferred)

We want to help members navigate the healthcare system in a better and safer way. The Transitions of Care (TOC) Team supports members who are newly discharged from the hospital (and in some cases prior to discharge). You’ll serve as an advocate for these members, coordinating care and ensuring they have the necessary resources and support to achieve better health outcomes. As a TOC Guide your goal will be to support members in safely recovering at home and to prevent hospital readmissions. Our ideal TOC Guide is caring, compassionate, solution-oriented and enthusiastic about providing an outstanding experience for Devoted Health’s members.
