What to Expect (Job Responsibilities):
- Assist registered nurses with scheduling and care coordination for patients in the Transplant Care Navigation program
- Communicate and coordinate with patients and care partners to ensure appointment attendance
- Document activities completed and update patients' medical records
- Utilize critical thinking and time management skills to facilitate patient scheduling
- Escalate issues to supervisors or management for resolution as necessary
What is Required (Qualifications):
- Associate’s degree required
- 1 - 2 years of related experience
- At least 1-year experience in a clinical setting preferred
- Ability to communicate effectively and professionally in a telephonic environment
- Must have a HIPAA-compliant workspace with high-speed internet
How to Stand Out (Preferred Qualifications):
- Scheduling training preferred
- EPIC proficiency preferred
- Prior remote employee experience strongly preferred
