**Job Title: Home Health Physical Therapist**
Join our team in Abilene, TX, for a 13•week contract, Monday to Friday, 40 hours weekly, no weekends or on•call.
**Responsibilities:**
• Deliver physical therapy in home health settings.
• Perform SOC assessments with Oasis.
• Create personalized care plans with patients and families.
**Qualifications:**
• Valid PT License and BLS certification.
• 1+ year of home health experience.
• Familiarity with HCHB and Oasis.
**Additional Information:**
• Potential for contract extension.
• Flexible scrub colors.
• Service area within 75 miles of Abilene, TX.
