- Minimum 2 years of acute hospital RN case management experience
- Active Compact RN License at time of submission
- BLS Certification
- BSW or higher (for SW roles)
- Critical care experience preferred
- Beds Covered: 36
- Staffing:
- 3 Social Workers
- 10 RN Case Managers
- 1 Manager and 1 Director
- Patient Ratios: 18:1 (RN CM), 25–40 (SW consults)
- Documentation Systems: Expanse, CarePort, Midas
- Equipment: Computer-based documentation
- Floating Policy: May float to other assignments as needed
- Dress Code: Business casual
- Strong leadership qualities
- Excellent communication and coordination skills
- Comfortable working with families and navigating complex discharge needs
- Team-oriented and proactive
- No weekend shifts for most roles
