Job Description & Requirements
2-3 year(s) of experience required Case Management (Utilization Review)
•Working with insurance daily, coordination and updating of clinical information for patients – speaking with insurances on the phone to confirm or deny that patients meet the criteria to be in the hospital•Discharge planning – working with ICU, Med/Surg, Women and Family, ER
•Utilization Review: help determine patients' status and appropriate level of care
•Case load: 8-20 patients daily
•Team: 4 case managers, manager often takes patient cases – social worker, case management assistant (UR Review – any in depth insurance questions)
•Work with transitional management who works with care coordination
•Skilled placement, rehab placement, home health placement, custodial placement
•Discharge needs: Walkers, crutches, oxygen, beds
•Work with our home health department and others depending on where the patient lives
•Nurse will go into patient room to create the discharge plan and determine needs
•Could coordinate transfer higher level of care – Renown, UC Davis, Stanford – arrange transport – ambulance, fixed wing, helicopter (2 – 5 times/month)
Licenses/Certifications:•California Registered Nurse (RN)
•Nevada Registered Nurse (RN)
•Basic Life Support (BLS) for Healthcare Providers (BLS: Employee will be enrolled in the Resuscitation Quality Improvement (RQI) Basic Life Support (BLS) Entry or Prep Curriculum (depending on their previous BLS certification)
Education:•Bachelor's Degree in nursing (BSN) with two years' experience in the acute hospital setting as a Case Manager.
•Will consider an Associate degree in nursing with five years' work experience in the acute hospital setting as a Case Manager.
•Case Management Certification
