Seeking a compassionate Hospice RN Case Manager to join a field-based hospice team providing individualized care to patients in their homes. Responsibilities include assessing patient/family needs, developing and managing care plans, coordinating with multidisciplinary teams, documenting interventions, and participating in performance improvement activities. Travel within assigned territory is required.Primary Territory: Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose, and surrounding cities.
EMR: Homecare Homebase (HCHB)
Weekends: One weekend per month required
Holiday Coverage: Required
