      What to Expect (Job Responsibilities):
      - Engage with individuals via phone or Telehealth to assess needs and collaboratively develop plans for connecting them to appropriate services
      - Provide excellent customer service and maintain accurate, professional documentation of all client interactions
      - Build and sustain relationships with facility staff to support follow-up care and post-discharge appointments for individuals
      - Understand and facilitate referrals to Mobile Crisis Outreach Teams across Washington State
      - Support program functions through participation in community events like health fairs

      What is Required (Qualifications):
      - Knowledge of Tribal communities and the Indian Health System
      - Agency Affiliated Counselor Registration required within 30 days of employment
      - Passing a typing test with a score of 35 WPM or higher
      - Fluency in English (oral and written)
      - Strong problem-solving and information-gathering skills

      - Proficiency in Motivational Interviewing and active listening
      - Ability to multitask, including talking and typing simultaneously
      - Efficiency in documenting notes in electronic medical records
