      Job Summary:
      Unit Clerks, under direct supervision, perform routine clerical functions in a medical office or clinical setting. The Unit Clerk supports the philosophy and complies with the protocols, policies and procedures of the Organization and the assigned Medical Office Department.

      Essential Responsibilities:
      • Upholds Kaiser Permanentes Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws.
        • Perform routine clerical, reception and appointment making functions following established protocols, policies and procedures within their scope of training and responsibilities.
          • Arrange for patient admission and transfer to acute hospital or other facility as directed.
            • Maintain files (tickler or log) for all Lab and other diagnostic test results. Arrange for appropriate disposition of results following established protocols, policies and procedures.
              • Expedite referral and consultation request - complete referral form, attach copies of required documentation (progress notes, EKGs, diagnostic test results, etc.), arrange appointments for STAT/Urgent request, etc.
                • Provide medical record information to providers as required - request charts, diagnostic test results, patient information from electronic medical record, records from outside contract providers, etc.
                  • Complete laboratory, X-Ray requisitions and other specialized test forms. Arrange appointments for STAT and urgent requests for specialized diagnostic tests (CAT scans, MRIs, ultrasound, etc.)
                    • Answer telephones and assist callers following established quality of service guidelines. Principle Job Duties and Skills:
                      • Schedule appointments for patients to follow up with primary care provider, nurse clinic, specialty clinic, etc.
                        • Make appointments for patients with STAT or urgent referrals.
                          • Contact member if not in clinic for appointment (referrals, primary care, diagnostic tests, etc.).
                          • Complete referral/consultation requests and attach appropriate documentation as indicated in referral guidelines (progress notes, diagnostic test results, EKGs, patient questionnaires, etc.).
                          • Contact Quality Resource Management case manager for approvals and authorization numbers on STAT and urgent referrals to outside contract providers.
                          • Fax patient information to internal/outside providers for appointments.
                          • Arrange for copies of X-Rays to be sent to consulting providers (with patient or via courier).
                          • Maintain department logs and files.
                          • File documents in medical records if chart available.
                          • Collect documents for loose filing and send to medical records (if chart not available). Order charts and other medical records as directed.
                          • Retrieve patient data and information from computer systems (lab results, progress notes form electronic medical record, etc.).
                          • Request documentation from outside contract providers (hospital, ER/urgent care, cardiology, orthopedics, etc.).
                          • Complete laboratory, X-Ray requisitions and specialized test forms as ordered by providers. Maintain files (tickler or log) for all laboratory and other diagnostic tests.
                          • Separate and collate (alphabetically by date) all lab, X-Ray and other diagnostic test results received daily for each provider.
                          • Give lab/diagnostic test results to providers for follow up.
                          • Send lab/diagnostic test results to providers for follow up.
                          • Send lab/diagnostic test result letters to patients after completion by provider.
                          • Contact hospital admitting to arrange for bed for patients being admitted.
                          • Contact hospital physician on duty or on call.
                          • Contact ambulance service to arrange for transport of patients to hospital.
                          • Copy patient information for ambulance, hospital and admitting physician.
                          • Transport patients to/from designated areas, and transport laboratory specimens.
                          • Answer telephones.
                          • Screen telephone calls and route to appropriate parties for disposition (providers, nurses, etc.). Maintain clean and organized work area.
                          • Order and maintain office and other non medical materials and supplies.
                          • Operate office equipment (copier, fax, computers, etc.)
                          • Miscellaneous Duties:
                          • Recommend procedure changes.
                          • Train/orient new or less experienced personnel.
                          • Participate in department meetings.
                          • Be familiar with regulatory requirements (Medi-Cal, Medicare, etc.)
                          • Assume other activities and responsibilities from time to time as directed.
      Basic Qualifications:
      Experience
      • 1-2 years experience in medical office/acute care facility required.
      • Computer and typing skills required, 30wpm.
      Education
      • High School Diploma or G.E.D. required.
      License, Certification, Registration

      Preferred Qualifications:
      • Knowledge of medical terminology preferred.
