What to Expect (Job Responsibilities):
- Document all calls and contacts and conduct follow-up activities if a visit is not scheduled
- Conduct a preliminary assessment of clinical referral information to determine eligibility for hospice care
- Communicate actively and effectively with interdisciplinary personnel
- Gather and document all required information to complete referral summaries
- Conduct virtual and/or telephonic triage through problem assessment and needs identification

What is Required (Qualifications):
- Graduation from an accredited school of nursing with a current license to practice as a registered professional nurse
- 1-2 years of varied nursing experience, with hospice experience preferred
- Strong clinical assessment skills related to hospice eligibility, pain and symptom management, and care planning
- Ability to use technology required of clinicians in their practice
- Excellent interpersonal skills, judgment, organizational skills, and attention to detail
