We are present in the full breadth of the continuum, from the patient’s home and virtual space, skilled nursing facilities, assisted living, and acute care hospitals.       Our family includes Intensivists, Oncologists, Geriatricians, Family Medicine, Emergency Medicine, Internal Medicine, Nephrologists and Hospice and Palliative Care Physicians and Nurse Practitioners.

      What You Need to Know

      IPC Weekend Visit Physician – Responsibilities

      • Display expertise in providing symptom management and ongoing goals of care discussions with our patients.
      • Possess excellent communication skills with an understanding of working with seriously ill and/or terminal patients and their families.
      • Work closely with the medical director, the team physicians, and the team leaders to provide continuity of care consistent with the hospice philosophy and the patient's wishes. Work effectively with all members of the hospice team.
      • When a physician provides visits in an inpatient setting, call responsibility is included in the rounding responsibility.
      • Complete all documentation within 24 hours of the visit.

      Qualifications

      Visit Physican Qualifications:

      • Maintains an active license and DEA in the state where care is being provided.
