- Under direction, ensure that the activities of this position and relevant programs are consistent
with the mission, vision and values of Catholic Charities, Diocese of Cleveland. - Make recommendations as to client’s employability and need for additional treatment or
support services. - Maintain confidential client records.
- Conduct interview and administer screening tools with client to assess vocational needs (i.e.,
gather information regarding client’s disability, functional limitation, work history, social history,
educational background and special interest) and prepare detailed report of interview. - Obtain medical/psychological information to make a recommendation regarding consumer
eligibility for services. - Secure the necessary diagnostic assessment documentation to identify functional limitations
to employment and make recommendations regarding the eligibility and order of selection
decision process. - Consult with other professionals regarding consumer needs/issues.
- Counsel eligible consumers toward increased functioning and mutually agreeable vocational
goals based on an assessment of interest, values, capabilities and vocational handicap. - Prepare a comprehensive assessment incorporating consumer informed choice,
medical/psychological information and outcomes from vocational rehabilitation goals. - Counsel client(s) on personal problems, career planning and emotional effects of disability and
develop individualized plan of services for each client, order rehabilitation services and
authorize their purchase. - Develop with eligible people with disabilities a comprehensive individual written vocational
rehabilitation program (IPE). - Initiate the vocational rehabilitation program via ORSC case management systems and
monitor ongoing vocational rehabilitation process. - Monitor client’s progress via review of progress reports, meetings and telephone contacts and
identifies service plan amendments or need for additional services. - Coordinate vocational rehabilitation services which may include medical/mental health
restoration services, job seeking skills training, job search and job coaching services. - Utilize community resources when available to assist in the vocational rehabilitation process
for eligible consumers. - Adhere to consumer confidentiality throughout the vocational rehabilitation process.
- Maintain case records according to funder policy and federal and state regulations.
- Document individual progress in funder’s case management system.
- Conduct case closure as required by individual consumer needs and policies of the funder.
- Perform other duties as necessary to accomplish objectives.
- Combination of experience and education normally represented by a Bachelor’s Degree in
Social Work, Rehabilitation Counseling or related field with 3-5 years related experience to
include Rehabilitation Counseling or closely related field and vocational assessment. - LSW preferred. but not required.
- Bilingual English/Spanish preferred.
- Must be able to form good working relationships.
- Must have competent oral, written and interpersonal communication skills.
- Must be able to effectively and productively work with people of very diverse economic,
cultural and administrative backgrounds. - Must be knowledgeable and responsive to cultural diversity issues presented in the
coordination of services. - Final applicant is required to be fingerprinted to complete background checks
