      As a discharge coordinator, you will play a pivotal role in ensuring patients have a smooth and timely transition from hospital to home. You will collaborate with a multidisciplinary team, including physicians, nurses, social workers, and therapists, to assess patients' needs, develop discharge plans, and coordinate care services.
