      Duties & Responsibilities

      • The Wraparound Supervisor ensures individualized planning, effective service coordination and management of care for all WAA enrollees including family participation and opportunities to identify appropriate goals, strengths, and needs in the development of risk assessments, crisis plans and plans of care.
      • Supervises and assists with planning activities. Provides guidance and consultation for Wraparound Facilitators as they work with child/youth, family (or the child/youth’s authorized healthcare decision maker) and others to identify strengths, needs and goals of the child/youth and family as needed.
      • Supervises no more than 8 Wraparound Facilitators and assumes liaison role between the Statewide Management Organization, Wraparound Facilitators and families in care planning and service coordination activities.
        • Observes CFT and engagement meetings on a regular basis and is responsible for completing a meeting observation form.
        • Reviewing all documentation such as POCs, contact notes, charts, family stories etc. and provides feedback to WAF on all documentation.
        • Administers brief and/or full competencies on all facilitators and uses the competencies as well as other available data to inform collaborative coaching plans with the facilitator.
        • Completes all HR functions including signing off on time requests, assigning new referrals, reviewing documentation and completing any corrective action plans.
        • Verifies and completes all flex fund requests as well as assists facilitator in extreme crisis with families.
        • Conducts regular group coaching sessions and collaborates with the Training Coordinator and Lead Coach to identify any needed trainings.
      • Encourages staff development and provides training opportunities to support Coordinated System of Care principles and values.
      • Provides leadership in establishing a cohesive multi-systemic alliance among community stakeholders to assure that staffing, billing and interagency processes are working effectively and efficiently.
      • Assumes monitoring and evaluative role involving all aspects of Wraparound Agency services including: fiscal processes, claims management, payment management, quality assurance and quality improvement.

      Qualifications

      • Master's degree from an accredit University or College in human services, social, or behavioral science required.
      • 3+ years of experience in the human services field required, preferably with different systems. 2+ years demonstrated success in management/supervision required. Demonstrated success in managing community-based programs. Must meet requirements for Wraparound Facilitator.
      • Provides 24/7 on call assistance to Program Staff. Must be able to work a flexible schedule including nights and weekends.
      • Must be able to meet requirements for Eckerd's Auto Insurance and be able to drive for business purposes. Must be capable of extensive driving and travel.

      Eckerd Connects' Wraparound Agencies are part of the Coordinated System of Care ( CSoC ) under the state's behavioral health managed care program that serves children and adults in Louisiana with behavioral health, mental health, and addictive disorders. Coordinated Systems of Care are national evidence-based models for coordinating services in a family-driven manner to ensure more positive outcomes for children, youth, and families. The model incorporates a broad array of flexible effective services and supports that is organized into a coordinated network and that integrates care planning and management across multiple levels and systems. Through this coordinated network, meaningful partnerships are built with families and youth at service delivery, management, and policy levels. Family-driven and youth-guided services are provided in the home as well as in community settings that are data-driven and outcome oriented. Treatment plans are individualized and strength-based, culturally, and linguistically competent, and integrated across all child-serving systems.
