      Description

      Purpose:

      As a member of the Adult and Youth Outpatient Team, the Youth Staff Therapist will provide office-based and telehealth services, including family and individual therapy to primarily youth and families, but may also include adults determined by need. Services consists of therapeutic interventions that facilitate healing, skill-building, identification and the use of natural supports, and the use of community resources. Must be available two evenings a week until 8:00 p.m.

      Work Conditions :

      This is a hybrid position, with services being conducted both in-person and through telehealth platforms. In-person services only provided in the C4 Clark office. Ability to work two evenings is required.

      Duties and Responsibilities:

      1. Provide individual and family therapy and case management services

      2. Develop treatment plans collaboratively with clients by defining goals, specific outcomes, and time frames for treatment

      3. Complete all necessary documentation (Mental Health Assessments, Treatment Plans, Progress Notes, etc.) in a complete and timely fashion, meeting agency standards/requirements.

      4. Work collaboratively with the Clinical Supervisor and other members of the Adult & Youth Outpatient team, as well as other community service providers, in order to coordinate and integrate all aspects of care.

      5. Identify crisis interventions when indicated.

      6. Adhere to confidentiality practices and procedures according to agency policies

      7.Maintain compliance at all times with agency policies and procedures

      8. Other duties as assigned by supervisor

      Qualifications


      Minimum Qualifications:

      1. Master’s Degree in counseling, marriage and family therapy, social work or related field required

      2. Experience in providing counseling services to children, families, and adults

      3. Availability to work evening hours required

      4. Knowledge and experience with Illinois Medicaid Comprehensive Assessment of Needs and Services (IM+CANS) preferred.

      5. Excellent interpersonal, verbal, and written communication skill
