Job summary
We are looking to recruit (1) Lead Clinical Pharmacist and (2) Clinical Pharmacist.
We are a large, friendly, progressive and dynamic GP practice , serving a list size of about 24,000 patients.
Main duties of the job
We are looking for Clinical Pharmacists who team players and can deliver exceptional patient care.
You should be able to perform patient assessments, create and implement personalised care plans, deliver evidence based treatments to optimise patient outcomes, maintain accurate patient records, prescribe independently in accordance with clinical guidelines, conduct face to face /telephone medication reviews , run specialist clinics, provide expert advice on prescribing and medicines management in primary care and support patients with long-term conditions with medication management.
For the role of Lead Clinical Pharmacist you must have prior management experience.

We are the lead GP practice in our local PCN and have extensive representation in our CCG.
