      Position Overview:
      Under the direction of the ACCESS Harris Program Director, the ACCESS Senior manager will lead cohort managers in administering innovative projects and programs across the County Safety Net Departments that address community health needs with meaningful, measurable, and sustainable public health impact. The ACCESS Senior Manager will ultimately be responsible for leading and managing a team of ACCESS Harris cohort managers and cohort/care coordination teams. The ACCESS Senior Manager will also ensure that the programmatic and administrative direction from the program director is successfully executed by the cohort teams. The position schedules and facilitates ACCESS Cohort meetings and care coordination sessions, as well as coordinates ACCESS Cohort activities with technology. In addition, the position provides regular reports and updates to stakeholders; works with Data Analytics teams to help inform system and program development; enhances and expands ACCESS Harris-related communication and collaboration between the safety net departments and community partners.

      Duties and Responsibilities:
      • Leads and develops a team of managers to meet program key performance indicators and benchmarks.
      • Establishes and tracks performance-based metrics to evaluate processes and interventions to inform quality improvement activities
      • Developing policies and procedures related to ACCESS Harris and the ACCESS Cohorts; advises cohort managers on them accordingly
      • Balances the various goals of partner agencies and the cohort teams with the needs of the Harris County community.
      • Develops and maintains records management system to provide proper evaluation, control, and documentation of assigned programs.
      • Develops a multi-disciplinary case-planning document template to guide services among agencies.
      • Develops and maintains a continuum of cohort-specific services for each unique participant group.
      • Maintains regular and effective communication between multi-disciplinary teams, program administration, and safety net agencies to build and maintain effective working relationships
      • Acts as an ACCESS Harris subject matter expert for the Houston/Harris County region and a central point of contact for coordinating services for these high need populations may need to coordinate and participate in presentations about the ACCESS Harris Initiative and the ACCESS Cohorts
      • Ensures the programmatic goals and direction received from the ACCESS Harris Program Administrator and Safety Net Collaborative are communicated and carried out through all cohort teams.
      • Performs other duties/responsibilities or special projects as necessary

      Education:
      • Bachelor’s degree in Business Administration, Public Health, Health Administration, Social Work, Nursing, or related field of study.
      Experience:
      • Five (5) years of professional experience in public health or related social service
      • Three (3) years of experience managing multiple teams with various priorities, goals, and KPIs

      Education:
      • Master of Science in Nursing (MSN), Master of Social Work (MSW), Master of Healthcare Administration (MHA), or related field of study
      Experience:
      • Three (3) years of experience in a supervisory or leadership role.
      Licensure/Certification:
      • Licensed Clinical Social Worker (LCSW)
      • Clinical Case Manager (CCM) certification
      Position Type, and Typical Hours of Work:
      • Full-Time | Grant Funded
      • Monday - Friday; 8 am - 5 pm
      • 40 hours per week (including evenings and weekends)
      Work Environment:
      • This job operates in a professional office environment.
      • This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
      Physical Demands:
      • This position is mostly sedentary but may occasionally require exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
