      Job summary

      The Wycliffe Medical Practice are seeking to employ a part time independently prescribing Advanced Nurse Practitioner or Advanced Clinical Practitioner, with the ideal candidate being someone who is proactive, enthusiastic and happy to work as part of an outstanding, long-established team. At the Wycliffe Medical Practice, we strive to deliver high quality, patient-centred healthcare and are looking for an advanced practitioner who will share our vision.

      Main duties of the job

      Key responsibilities will be:

      To assess, diagnose, plan, implement and evaluate treatment/interventions.

      Diagnose and manage both acute and chronic conditions.

      Clinically examine and assess patient needs and plan clinical care accordingly.

      Care for patients presenting with an undifferentiated diagnosis and be able to refer appropriately.

      Support patients to adopt health promotion strategies that promote healthy lifestyles

      Demonstrate appropriate communication ensuring patients are fully informed and consent to treatment

      The ideal candidate will also have:

      Applicants should be autonomous practitioners with experience in primary care and minor illness.

      Able to independently Prescribe.

      at least 2 years experience of working as an ANP or ACP within General Practice

      Minor Illness and Injury qualifications

      A Knowledge of SystmOne, Clarity Teamnet and would be an advantage however being a Fast learner with the ability to learn a variety of IT based systems is essential.
