What to Expect (Job Responsibilities):
- Provide medication management, including medication reconciliation and adherence counseling
- Collaborate with the interdisciplinary care team to develop individualized care plans
- Monitor and evaluate medication regimens to ensure clinical criteria are met
- Identify opportunities for drug therapy optimization and recommend cost-saving initiatives
- Advocate for patient safety and medication literacy through education and counseling
What is Required (Qualifications):
- Doctor of Pharmacy (PharmD) and licensed pharmacist in WA
- Completion of PGY1/PGY2 residency preferred, but not required
- Strong verbal communication skills and deep knowledge of pharmacology
- Ability to analyze information, exercise good judgment, and maintain legal compliance
- Organized, dependable, and empathetic with a focus on quality and detail

- Experience in disease state management and treatment guidelines
- Energetic and highly motivated team player with strong personal and communication skills
