      What to Expect (Job Responsibilities):
      - Interact with low stratification members via phone calls to coordinate care and update assessments and care plans
      - Create and maintain individualized care plans based on assessments and claims data
      - Participate in interdisciplinary team meetings to discuss members' health care needs and barriers to care
      - Identify and link members with health plan benefits and community resources
      - Perform administrative tasks to maintain required skills for job duties

      What is Required (Qualifications):
      - Minimum of 3+ years of experience in a health-related field
      - Strong organizational skills and the ability to work independently
      - Excellent verbal and written communication skills
      - Proficient with computer systems and knowledgeable in Microsoft Office Software
      - Strong customer service skills

      How to Stand Out (Preferred Qualifications):
      - CRC, CDMS, CRRN, COHN, or CCM certification
      - Experience with Medicare and Medicaid
      - Managed care experience
      - Experience working with geriatric special needs, behavioral health, and disabled populations
      - Bilingual (English/Spanish; English/Creole)
