What to Expect (Job Responsibilities):
- Conduct routine care coordination, support, and education for members enrolled in the Dual Special Needs Plan
- Develop proactive care plans to enhance short and long-term health outcomes
- Identify high-risk factors and coordinate appropriate referrals for clinical case management
- Monitor care plan progress and consult with multidisciplinary teams to achieve optimal outcomes
- Utilize motivational interviewing skills to engage members and promote healthy lifestyle choices
What is Required (Qualifications):
- Minimum of 3 years of experience in a health-related field
- Bachelor's degree or equivalent experience required
- Practical knowledge of case management processes and best practices
- Exceptional communication and negotiation skills
- Ability to work collaboratively with healthcare providers and community organizations
How to Stand Out (Preferred Qualifications):
- CRC, CDMS, CRRN, COHN, or CCM certification
- Experience with Medicare and Medicaid populations
- Managed care experience
- Familiarity with geriatric special needs, behavioral health, and disabled populations
- Bilingual (English/Spanish or English/Creole)
