      Job summary

      Advanced Nurse Practitioner - Community Mental Health Team

      The service offers support and treatment to service users experiencing moderate to severe mental illness, in a community setting.

      Role requires full Non-medical prescribing/ ANP qualifications

      Main duties of the job

      The post holder will work as part of a multi-disciplinary team delivering care to service users in a range of community settings. Other members of the team include Consultant Psychiatrists, Hub Manager, Clinical Psychologists, Clinical Leads, Assistant Psychologist, Mental Health Nurses, Occupational Therapist, Well-Being Practitioner, Art Therapist, Medical Support Workers and STR workers. The ability to work flexibly in multidisciplinary teams and in a range of settings is therefore essential.

      The post holder will, in conjunction with the management team, provide strong clinical leadership and expertise to the multidisciplinary team, ensuring that high standards are maintained.

      The Advanced Nurse Practitioner will hold a caseload and will manage regular follow up and assessment clinics. They will work closely with the medical team to ensure effective patient flow and discharge management is maintained.

      Applicants must be highly motivated, have a genuine interest in mental health and have experience in mental health care. .
