Handle scheduling requests specific to Collaborative Care services.
Complete Intake Initial Contact Forms (ICFs) accurately and promptly.
Conduct client callbacks for scheduling and follow-up inquiries.
Assist in receiving and reviewing incoming referrals to ensure they are complete, accurate, and meet necessary criteria for assessment and evaluation.
Support prioritization of referrals based on urgency and established guidelines or protocols.
Help coordinate and schedule assessments, ensuring appointments are scheduled in a timely manner and that clients receive necessary instructions and paperwork prior to their assessment.
Maintain regular and effective communication with referring healthcare providers, clients, and members of the assessment team.
Provide updates on the status of referrals, schedule appointments, and answer inquiries regarding the referral process.
Collaborate with assessment team members to facilitate efficient workflow and ensure the availability of necessary resources.
Maintain accurate and organized documentation of all referral-related information, including referral details, communication records, and scheduling information.
Adhere to privacy and confidentiality regulations when handling sensitive client information.
Monitor and track the progress of referrals through the assessment process.
Identify any delays or bottlenecks and take appropriate actions to resolve issues and ensure smooth operations.
Utilize electronic health record systems or databases to track referrals, assessments, and outcomes.
Learn to maintain and update a directory of behavioral health providers, social services, and community resources.
Receive training on referral tracking systems to ensure clients receive services and follow-up care.
Develop skills to build collaborative relationships with partner agencies and stakeholders.
Train in triaging clients based on risk levels (e.g., suicide risk, homelessness, acute mental health crises).
Acquire skills for conducting safety plans and following emergency protocols in high-risk situations.
Learn de-escalation techniques and client-centered crisis intervention strategies.
Receive training in documentation standards and software for managing client records and referral tracking.
Develop the ability to analyze data to measure referral success rates, wait times, and identify service gaps.
Understand and ensure compliance with relevant policies and regulations.
High school diploma or equivalent required.
Must currently hold or be actively pursuing certification as a Behavioral Health Aide or Chemical Dependency Counselor.
Clinical competency required within 3 months and every 3 years
Minimum 2 years of experience in a healthcare or mental health setting, particularly in administrative support, referral management, or care coordination.
Assessment and referral processes, intake procedures, and diagnostic criteria.
Familiarity with electronic health record systems and data management.
Regulatory requirements and privacy regulations related to assessment and referral processes.
Excellent organizational and time management skills to manage multiple tasks and prioritize effectively.
Strong communication and interpersonal skills to interact with team members, clients, and external stakeholders.
Attention to detail and the ability to maintain confidentiality.
Proficient in Microsoft Office Products including Word, Excel, and PowerPoint
