      What to Expect (Job Responsibilities):
      - Collaborate with healthcare team members and community service agencies to coordinate Behavioral Health and PUMS programs
      - Assess barriers to compliance with medical care plans and implement interventions to promote compliance
      - Act as a resource to assist members with community-based programs for Mental Health and Substance Abuse
      - Empower members to navigate the process for obtaining appropriate medical care and community assistance
      - Work with the Pharmacy Department to manage prescription services for PUMS members

      What is Required (Qualifications):
      - Bachelor's degree in social work, Health and Human Services, or a related field with 15 credit hours in health and human development
      - Minimum of 3 years of related experience in a physician office, Community Services Board, acute care, or managed care setting
      - Direct experience working with patients related to Behavioral Health or Substance Abuse treatment

      How to Stand Out (Preferred Qualifications):
      - Experience in a community-based behavioral health setting
      - Knowledge of medication management and chronic pain treatment strategies
      - Familiarity with community resources for mental health and substance abuse
