What to Expect (Job Responsibilities):
- Collaborate with healthcare team members and community agencies to coordinate Behavioral Health and PUMS programs
- Assess barriers to compliance with medical care plans and implement interventions
- Assist members in navigating community-based programs for Mental Health and Substance Abuse
- Work with the Pharmacy Department to manage prescription services for PUMS members
- Empower members with skills to obtain appropriate medical care and assistance
What is Required (Qualifications):
- Bachelor’s degree in social work, Health and Human Services, or a related field with 15 credit hours in health and human development
- Minimum of 3 years of related experience in a physician office, Community Services Board, acute care, or managed care setting
- Direct experience working with patients in Behavioral Health or Substance Abuse treatment
How to Stand Out (Preferred Qualifications):
- Experience in a remote work environment
- Additional certifications related to Behavioral Health or Substance Abuse
- Familiarity with community resources for mental health and substance abuse services
