      Remote Care Specialist...LPN or RMA/CMA...BILINGUAL (POLISH SPEAKING)

      Job Summary We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will be responsible for assisting in the coordination of patient care services, ensuring smooth operations within the healthcare facility.

      Duties

      • Coordinate patient appointments and follow-ups
      • Assist in managing medical records and documentation
      • Communicate with healthcare providers, patients, and families
      • Ensure compliance with healthcare regulations and protocols
      • Work closely with other licensed professionals and physicians to support patients remotely
      • Assist in creating personalized care plans for each patient

      Requirements

      • Certified / Registered Medical Assistant or Licensed Practical Nurse (active and in good standing)
      • Must have experience working with patients on chronic care management
      • Ability to sit for long periods of time and talk on the phone with patients
      • This is a work from home position. You will need to have a home office set-up with high-speed internet, computer and phone. The space must also be a quiet area.
      • Strong communication skills
      • Ability to work in a fast paced environment
      • Friendly personality and lots of patience
      • Remote Patient Monitoring Experience is a plus
      • Fluent in Polish required.
