      Additional Information About the Role Full-Time; benefits eligible! Monday - Friday, 8am-4:30pm, onsite. Must have a Bachelor's degree with at least 5 years of experience in a healthcare/hospice setting with experience in admissions, business development and eligibility. Overview BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs. BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country. Preferred Qualifications Role Purpose This role works with internal & external referral sources to facilitate hospice growth and quality metrics for all hospice locations. Provides support to the Hospice Director to identify areas of opportunity and propose solutions using data analysis. Responsible for managing, in coordination with HSO case management and external referral sources, the safe & effective transition of patients onto Hospice services. Responsible for identifying, initiating, developing, and managing business relationships with hospitals, physicians, and post-acute care settings. Coordinates and executes objectives identified by the organization to reach both short term and long-term range growth plans. Ensures department practices support agency, state, and federal regulations, ensuring high reliability and financial processes. Responsibilities Recommends, develops, updates strategic business plans to support organizational practices and goals in conjunction with the Hospice Marketing Liaison(s). Formulates and defines business scope and objectives through research and fact-finding combined with an understanding of business and industry requirements. Ensures the highest quality of care in compliance with local, state & federal regulations. Ensures patients receive quality care by communication, coordination, and documentation of clinical, demographic and payor information. Coordinates and executes objectives identified by the organization to reach both short term and long-term range growth plans. Facilitates hospice growth and quality metrics and supports Hospice Director to identify areas of opportunity. Manages individual(s) including but not limited to: onboarding and hiring, training, workload management & performance evaluations, conducting professional development plans as needed. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership. Develops and adheres to departmental staffing, revenue and/or expense budgets. Responds to changes in the business which may affect the ability to achieve the budget goals. BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
