      What to Expect (Job Responsibilities):
      - Perform outreach to members via phone to support care plan next steps and address questions or concerns
      - Serve as the front-line support for various member and provider inquiries, explaining care plan procedures and protocols
      - Document and maintain non-clinical member records in accordance with state and regulatory requirements
      - Assist with member onboarding and administrative duties, including sending welcome letters and educational materials
      - Connect members to health plan and community resources to ensure high-quality customer service

      What is Required (Qualifications):
      - High School diploma or GED
      - 1 – 2 years of related experience
      - Ability to handle telephonic interactions with members effectively
      - Knowledge of existing benefits and resources to address Social Determinants of Health (SDOH) needs
      - Compliance with all policies and standards

      How to Stand Out (Preferred Qualifications):
      - Experience in a healthcare-related customer service role
      - Familiarity with care management activities and health plan resources
      - Strong communication skills and ability to navigate complex inquiries
