What to Expect (Job Responsibilities):
- Answer inbound calls to assist members with completing assessments and recommend a plan for services based on member needs
- Make outbound calls to follow up with members regarding their care services
- Help members and families access community resources, including homecare, meals, transportation, and social support services
- Participate in the coordination and scheduling of services for members
- Perform administrative tasks to support members' planning and execution of their benefits

What is Required (Qualifications):
- Must be 18 years of age or older
- High School Diploma preferred
- Proven experience answering inbound calls
- Excellent written and verbal communication skills
- Ability to work with multi-disciplinary teams and thrive in a fast-paced, dynamic environment

How to Stand Out (Preferred Qualifications):
- Experience in Public/Community Health or related field
- Background in Social Services, Care Coordination, Healthcare Navigation, or Community Organizing
- Strong organizational and time-management skills
- Passion for improving health outcomes through community-based services
