      What to Expect (Job Responsibilities):
      - Conduct in-person and remote assessments of members with serious mental illness
      - Provide case management services to patients, families, and designated caregivers
      - Develop and monitor multidisciplinary collaboration of services to patients
      - Assist adult patients and their families with personal and environmental challenges
      - Engage in community involvement and support programs for members with chronic illnesses

      What is Required (Qualifications):
      - Must possess a degree in Health & Human Services or a related field
      - Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
      - At least one year of mental health experience preferred
      - Degrees must be from accredited institutions listed on the U.S. Department of Education College Accreditation database
      - Must reside in South/Southwest Central Virginia and surrounding areas

      - Experience in mental health services or case management
      - Familiarity with community health resources and support programs
      - Strong interpersonal and communication skills
      - Ability to work collaboratively within a multidisciplinary team
      - Knowledge of healthcare regulations and compliance
