      What to Expect (Job Responsibilities):
      - Perform case management services for patients, families, and designated caregivers
      - Develop and monitor multidisciplinary collaboration of services for patients
      - Assist adult patients and their families with personal and environmental difficulties related to medical conditions
      - Conduct both in-person face-to-face assessments and remote telephonic assessments for serious mental illness
      - Provide support to patients during terminal illness situations

      What is Required (Qualifications):
      - Degree in Health & Human Services or a related field from an accredited institution
      - Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
      - At least 1 year of mental health experience preferred
      - Must reside in Norfolk, VA Beach, Suffolk, Chesapeake, Portsmouth, VA, or surrounding areas
      - Certification or licensure is not required

      How to Stand Out (Preferred Qualifications):
      - Experience in mental health services or case coordination
      - Familiarity with community health resources and programs
      - Understanding of Medicaid and health plan operations
      - Skills in multidisciplinary collaboration and patient advocacy
      - Knowledge of serious mental illness treatment approaches
