What to Expect (Job Responsibilities):
- Conduct face-to-face and telephonic assessments for patients with serious mental illness
- Provide case management services to patients, families, and caregivers, addressing personal and environmental difficulties
- Develop and monitor multidisciplinary collaboration of services for patients
- Assist adult patients and their families during terminal illnesses and other medical conditions
- Maintain accurate documentation and manage casework duties effectively
What is Required (Qualifications):
- Must possess a degree in Health & Human Services or a related field such as Psychology, Social Work, or Nursing
- Minimum of 1 year of experience in Long Term Care, Health Plan, and Medicaid
- At least 1 year of mental health experience is preferred
- All degrees must be from accredited schools recognized by the U.S. Department of Education
- Must reside in Norfolk, VA Beach, Suffolk, Chesapeake, Portsmouth, VA, or surrounding areas
How to Stand Out (Preferred Qualifications):
- Experience in case management or care coordination within a healthcare setting
- Familiarity with behavioral health services and community resources
- Knowledge of Medicaid regulations and health plan operations
- Strong interpersonal and communication skills to effectively work with diverse populations
- Ability to handle sensitive situations with empathy and professionalism
