What to Expect (Job Responsibilities):
- Provide outreach to members via phone or home visits to discuss care plans and resources
- Coordinate care activities with healthcare providers and community partners
- Address member and provider inquiries related to care plans
- Document and maintain member records in compliance with regulatory requirements
- Identify needs and make referrals to care managers and community organizations
What is Required (Qualifications):
- High School diploma or GED
- 1-2 years of related experience in care coordination
- For Arkansas Total Care plan, a Bachelor's degree in a social science/health-related field or relevant experience with developmentally or intellectually disabled clients
- Valid driver's license and completion of child and adult maltreatment checks
- Ability to travel up to 30%

- Experience working with members with intellectual and developmental disabilities
- Previous field-based experience in healthcare services
