What to Expect (Job Responsibilities):
- Provide outreach to members via phone or home visits to discuss care/service plans and address questions or concerns
- Coordinate care activities in collaboration with healthcare providers and community partners
- Serve as a resource for member and provider inquiries related to care plans
- Document and maintain member records in compliance with state and regulatory requirements
- Provide education on available benefits and resources to members
What is Required (Qualifications):
- High School diploma or GED required
- 1-2 years of related experience in care coordination or healthcare services
- For specific plans, a Bachelor's degree in a social science/health-related field or relevant experience coordinating care for special populations
- Valid driver's license and ability to pass background checks and drug screens
- Must reside in Arkansas or a bordering city, with travel required
How to Stand Out (Preferred Qualifications):
- Experience coordinating care for developmentally or intellectually disabled clients or behavioral health clients
- Knowledge of community resources and healthcare systems
